Medical Records


Any patient wanting copies of medical records or if you are requesting for information be sent to your physician must stop by the medical records department and sign an R.O.I. before anything can be released. We also offer expectant mothers the option to pre-register for the Birth Certificate and Acknowledgement of Paternity (if applicable). You may stop by the medical records department and fill out the paperwork or if you have questions regarding the AOP process.
