Hospital Case Manager - Case Management
Location: Carrizo Springs, Texas

Job Title: Hospital Case Manager

Job Summary: The Hospital Case Manager at Dimmit Regional Hospital is responsible for coordinating patient care in the Emergency Room, Medical Surgical, and Swing Bed departments. Collaborating with physicians, nurses, social workers, and other healthcare professionals, the Case Manager ensures the delivery of high-quality, cost-effective care. This role involves assessing patient needs, developing and implementing care plans, and monitoring patient progress throughout their stay, requiring strong communication, organizational, and critical thinking skills.

Responsibilities:

• Conduct initial assessments of patients' medical, social, and psychological needs upon admission.
• Develop individualized care plans in collaboration with the healthcare team.
• Coordinate with various professionals to implement care plans effectively.
• Monitor and evaluate patients' progress, adjusting care plans as necessary.
• Provide education and support to patients and their families regarding care and discharge planning.
• Coordinate post-discharge care and services to facilitate patient transition.
• Maintain accurate and up-to-date documentation of patient assessments and care plans.
• Participate in quality improvement initiatives.
• Adhere to hospital policies, procedures, and regulatory requirements.
• Occasionally travel to visit local patients requiring in-home assessments or follow-up care.

Additional Responsibilities:

• Evaluate patients and identify appropriate level of care using InterQual Review Manager Software.
• Coordinate referrals and appointments for outpatient services.
• Follow transfers and referrals outside the system to ensure care services are directed back to the healthcare system.
• Perform follow-up calls on patients seen in the Emergency Department.
• Participate in IDT meetings and business office/case management meetings.
• Perform coverage for inpatient and swing bed case management when the current case manager is absent.
• Utilization review is performed by evaluating the effectiveness of care given by health care team members, including self, as reflected in the documentation of patient's progress/responses, communication skills and problem identification and resolution.
• Recommends the observation and inpatient admission process.

Qualifications:

• Current licensure as a Licensed Vocational Nurse (LVN) or Registered Nurse (RN) in Texas.
• Minimum of 2 years of clinical experience in an acute care setting, preferably in case management or care coordination.
• Certification in Case Management (CCM) preferred.
• Strong knowledge of medical terminology, disease processes, and treatment options.
• Excellent interpersonal and communication skills.
• Ability to work independently and collaboratively as part of a team.
• Proficiency in electronic health record (EHR) systems and Microsoft Office applications.

Physical Requirements:

• Ability to stand, walk, and move around the hospital and patient rooms for extended periods.
• Ability to lift and carry up to 25 pounds.
• Visual and auditory acuity to assess patient conditions and communicate effectively.
